If you are unhappy about the outcome of a complaint you can appeal the decision by submitting this form.
Complete the appeal form below if you would like to appeal a decision made by the Practice Standards Panel. If you are unhappy with the outcome of a complaint we can refer to an Independent Appeals Officer who will, initially, consider whether there are grounds for appeal. The process and grounds for appeal are outlined in complaints procedure.
Your appeal should be about the concerns you raised in your complaint and not about any new concerns you may now have about the member.
