Collaborative Practice Forum – Participation Agreement questions

Angela Lake-Carroll will be hosting a session at the upcoming Collaborative Practice Forum on the new Participation Agreement and guidance.

If you have a question about the new Agreement, please submit it here. We will try to take as many questions as possible although we may not be able to answer them all on the day due to the volume of people attending.
